Cheek Augmentation

A large portion of my work involves facial augmentation. I discussed the subject of chin augmentation in a previous blog post. This blog will be devoted to cheek augmentation for the rest of it.

Both nonsurgical and surgical methods of cheek augmentation are available. Dermal fillers or autologous fat—the patient’s own fat—can be used for nonsurgical facial augmentation. A more permanent method of improving and adding volume and support to the cheeks and surrounding soft tissues is through cheek augmentation with a facial implant. Additionally, a cheek augmentation causes a slight “lifting” of the jowls and face.

Surgical Procedure

A variety of synthetic materials are used to make cheek implants. The two most popular varieties are porous polyethylene and silicone-formed implants. The polyethylene implants have a textured surface, while silicone implants are typically smooth in composition. In my practice, I use both, but when it comes to cheek augmentation, I like the textured implants. I sometimes request a radiograph before surgery to assess the cheekbone symmetry or to rule out any bony pathology.

In my practice, either local or general anesthesia combined with sedation is used to place the implants. I would rather make an oral incision to implant the implants. Inside the mouth, where the upper teeth and inner cheek meet, I make an incision. I make a pocket between the muscles of the face and the cheekbone. To choose which permanent implant to use, I usually use a sizer, which is a temporary implant that fits into the pocket. To decide in the operating room which implant best enhances the patient’s appearance, I can arrange a variety of sizes and shapes of sizers on the right and left. In order to secure cheek implants to the underlying bone and stop them from rotating or moving, I sometimes insert a permanent titanium screw through the implant. Shortly after the oral incision is sutured shut, the patient is sent home.

Postoperative Care

Following surgery, a course of oral antibiotics is prescribed. Sometimes, in order to minimize swelling and avoid a postoperative hematoma, these patients are put into a compression facial garment. Moderate cheek bruising, swelling, and tenderness are common right after surgery. For the first 48 hours, you will be told to keep a soft diet and refrain from smoking while applying ice to your cheeks on an intermittent basis. Immediately following surgery, it is typical for the upper lip area to move slowly and numbly. Nerves “stretching” in this area is the cause of this. All healing takes time, and it may take two to three months for the swelling, bruises, and numbness to completely go away without any problems.
