When people are considering facelift surgery, one of the most common questions they ask is, “How long will it last?” Surprisingly, there is a dearth of scientific literature that allows physicians to provide an honest and direct response, so they often guess a number.
It is challenging to study patients over an extended length of time because, once they are satisfied with their procedure, they typically do not come back. It is also challenging to make comparisons from photos because they must all be taken in the same manner. Since 2022, I have taken pre- and post-operative photos of every patient using a standardized photographic system. I performed more than 1800 deep plane facelifts between 2012 and 2022. We discovered 55 of these patients who had returned, on average, six years after their facelift procedure for unrelated reasons, such as seeking advice regarding a different aspect of their appearance, by searching through my photo archive. I was able to take new pictures of them because of this. Therefore, the 55 patients for whom we had photos taken prior to their facelift surgery, at routine follow-up between four and seven months, and then at longer-term follow-up at an average of six years after their operation served as the basis for my recent study regarding the duration of a facelift. We aimed to assess how well the changes had been sustained over an extended period of time and to quantify changes in different facial features brought about by the procedure.
Using a cutting-edge computer program, we were able to mark important facial features, like the neck and jowl, and track how those features changed in position following a facelift. The jowl, mouth to chin (marionette) lines, nose to mouth (nasolabial) lines, and the angle between the neck and chin all showed notable improvements. Upon examining the long-term follow-up photos, we discovered that the favorable alterations in the lower face stayed consistent throughout the lengthy (on average, six years) follow-up period. Although it was still better, the neck moved closer to its pre-operative state than the other areas we measured. Patients frequently inquire as to whether their mouth will get wider, but neither the width of the mouth nor the width of the nose changed. The mouth’s corner has a tendency to slightly move upward; this change is also sustained.
In addition to the computer program, we employed two evaluation methods in which two laypeople, an aesthetic nurse, and another plastic surgeon were shown photographs to see what changes they believed had taken place. The photos were cropped to eliminate hair, clothing, and other elements that can significantly affect how a person interprets a photograph in an effort to minimize error. In essence, the observations matched the changes that the computer measurements had indicated.
We can now reassure patients that deep plane facelift surgery will improve their appearance and make them happier, with changes that will typically last for at least five and a half years. It won’t cause negative changes to the nose or mouth.
We intend to prolong the study period in the future, and it’s possible that these beneficial changes will last for ten years or longer. However, we will need to wait for proof that goes beyond the current five and a half years.
