Pain After Surgery
The number one question I get about labiaplasty is, “How painful will my labiaplasty be?” The area is very sensitive and full of nerve endings, but you might be surprised at the answer!
Pain after labiaplasty is mostly limited to the first night after surgery! You’ll be uncomfortable and swollen for a week or two, but the pain is really just the first night after surgery. The pain is usually described to me and my nurses as a pinching or pulsating/throbbing type of pain. Ice is the best treatment for this and our top recommendation for labiaplasty post op pain. Ice packs decrease swelling, stop more blood from flowing to the area and cause a little local numbness. For pain control during labiaplasty while awake we suggest 20 minutes on and 20 minutes off.
One great way to do this is to freeze a bottle of water and place it between your legs while you sit and watch TV or read a book. The water bottle โtrickโ can also be used when you go back to work to be more discreet when icing your labiaplasty healing area.
Ice can cover most of the discomfort and we also prescribe pain medication. Most of the patients either take it the first night only, or donโt take any at all! Many people are fine with just Tylenol and ice for the first night after labiaplasty surgery.

Labiaplasty Recovery Timeline
Everyone is different and some people will recover more quickly than others. But hereโs a rough timeline of what to expect after surgery to help you keep track of your progress.
Day One
Most likely youโll be awake for the surgery (96% of our patients drive themselves to the surgery center and drive themselves home with no anesthesia other than the local numbing injections) you can leave right after. You will receive non-stick soft dressings and an ice pack, and you will have your prescriptions from your pre-op visit. You are going to be most uncomfortable that first night so keep icing and have your pain meds on the ready.
Day Two and Three
It is normal to have some spotting at this time. A cold compress applied on days two and three can ease swelling and pain of the skin. Swelling usually reaches its maximum on the third day after labiaplasty.
Day Four and Five
By about the fourth or fifth day after your operation you should be seeing the swelling going down, but you could be seeing more bruising discolouration. During this time you can walk around lightly for short periods of time. Not ideal, but many patients do return to work or school on a limited schedule. Continue to use a cold pack to help reduce swelling and pain.
Day Six and Seven
This is typically when a follow-up appointment occurs to review your progress and concerns. Here we can see some differences between each side depending on how the swelling may resolve differently on each labia.
Week Two
Light walking is encouraged and you can continue with your medication. You can stop with the cold compresses as the swelling should show a significant reduction during the second week. Your stitches are all internal and dissolve at this point, but you won’t see them.
Week Three
This is around the time that you should be comfortable to sit as well as longer walks. Many people will also be able to begin light exercises in the third week.
Week Four to Six
By the fourth week you should be able to go back to your normal routine and exercises. At 5 weeks after the operation, there are no restrictions, so tampon use and bathing are okay. After labiaplasty you can have sex at 5 weeks post op but please listen to your body. There will be lots of new sensations and you want to make sure that your partner is very gentle with you.
Tips for a Healthy and Faster Recovery
- Expect to do nothing the first 2 days.
- Wear a baggy skirt or pants.
- White cotton underwear is preferred. No lace, no dyes, very soft.
- In the first week or two, ice packs and cold compresses are useful.
- You should not have sex or use tampons for at least six or seven weeks, or until you are completely healed.
- After using the bathroom, wash yourself with warm water and pat dry (weโll give you a recovery kit with everything you need!).
- Sleep enough at night and rest as much as you can for the first two weeks.
- Eat good food.
- No smoking or drinking.
- Watch out for yeast infections, we give everyone diflucan just in case!
How Does Labiaplasty Affect Intercourse and Exercise?

Labiaplasty and Sexual Intercourse
The labia minora is a very sensitive tissue with lots of nerve endings. That said, it is also one of the fastest healing places in the body. You know what itโs like to bite your tongue or cheek and it hurts at first? And also how fast the mouth gets better and how quickly that pain goes away? The same goes for labiaplasty. The tissues of the inside of the mouth and lips are almost identical to the tissues of the labia and vagina.
So, the labia minora are very sensitive initially after labiaplasty, but the healing is very fast. With that said, sex involves repeated friction of the labia. This is obviously not advised for any healing incision for all the obvious reasons. For these reasons it is important that you are absolutely sure that you are fully healed before you resume sexual activities after labiaplasty. I would recommend women wait 6 weeks to have sexual intercourse after having labiaplasty for safety.
Complications of Resuming Intercourse and Exercise Too Soon
Super common post-exercise, especially doing cardio, is repeated friction. Weightlifting is totally okay at three weeks post labiaplasty, but things like Peloton, elliptical, stairmaster, and running/treadmills are the worst. Repeated trauma and friction can cause damage to the repair of the labia minora and the suture line to separate, a process called dehiscence. Usually the healing from labia separation is totally inconsequential but can sometimes require a revision surgery. Because you wonโt be able to revise for several months after surgery, take it slow. Too much activity too soon can also prolong swelling and pain. This could also mean a longer recovery period, in turn. So take it easy!
How Long Should You Wait?
This is worth remembering, as every person is different. Some heal faster, while some may have to wait a few more weeks for the labiaplasty surgery to heal completely. I would say labiaplasty surgery 3 months is a time period of time where 100% of my patients are 100% recovered.
So please, if you want to exercise after labiaplasty, avoid cardio and consider light weights no sooner than 3.5 weeks post op. You can have sex 5 weeks after labiaplasty if your partner is gentle and you feel everything is right. Downtime For Desk Jobs After Labiaplasty
Depending on their comfort and the nature of their recovery, most patients can return to desk-based work in three to seven days. Sitting for long periods can be a bit uncomfortable at first, so standing up and walking around for a few minutes or using a soft cushion when needed can help with comfort. Jobs that involve heavy lifting, prolonged standing or strenuous physical activity may require a longer recovery period. Your surgeon will let you know when it is safe to return to your normal work schedule based on your healing progress.
