It’s never too soon to learn about this plastic surgery technique, as for some qualifying patients, the reconstructive work can be done at the same time as your lumpectomy or mastectomy.

How Long Will My Breast Reconstruction Take?

There are some rare exceptions to this. One is a technique called direct-to-implant (or DIT) reconstruction. In this case, a definitive implant is inserted at the time of the mastectomy. This sounds promising in theory but very few women are candidates for this technique and there are associated risks. Therefore it is unlikely to be proposed.
Another exception is some tissue-based breast reconstructions that do not use an implant or require a tissue expander. If your breast mound is being reconstructed with your tissue alone (DIEP flap technique) and you do not need an expander, the breast can be reconstructed in less time.
What Are My Breast Reconstruction Options?
Women who are planning to undergo breast reconstruction will learn about two different types of techniques: implant-based reconstruction and tissue-based reconstruction. Some women choose one or the other; others mix both.
Implant-Based Reconstruction
Theoretically, this technique works in much the same way as traditional breast augmentation, using a synthetic implant to create breast volume. They are similar but there may be some differences in appearance as there is less tissue covering the implant.
Implant based reconstruction is nearly always done in stages. First, a tissue expander is placed in the breast. This expander will be gradually filled (over several visits) to gently stretch the skin and tissue and prepare the body to receive (and support) the final implant.
After the tissues have expanded the expander is removed and the implant is placed.
This can be combined with tissue based reconstruction or fat grafting to increase the amount of natural tissue in the breast.

Tissue-Based Reconstruction
Tissue (called flaps) from many parts of the body can be used to rebuild the breast mound. One of the most commonly used flaps is called the DIEP flap. This tissue is from the abdomen (between the belly button and pubic bone).
In a tissue-based reconstruction, tissue from the abdomen is moved to the breast and the blood vessels are reconnected.
This technique can be done alone or with an implant for more breast volume. This technique does not always require the use of a tissue expander, but women who choose to delay their breast reconstruction may still require one.
When Can I Start My Breast Reconstruction?
As noted earlier, some women can start their breast reconstruction at the same time as their mastectomy, while others wait months or years for the surgery.
Women who delay their procedure don’t have an expiration date. You get to decide when you want to have reconstruction. Maybe this is when you have finished your treatment regimen and been given a clean bill of health; maybe this is when you have gotten your head around what you and your body have just been through; or maybe this is years down the line when you are in remission.
This could be one way for women who want to get involved in reconstruction. Tissue expanders are often placed or flap surgery is performed at the time of mastectomy.

What About Nipple Reconstruction?
Some women have non-nipple-sparing mastectomies, and their nipples are removed at the time of surgery.
