What is pre-vaginaloplasty orchiectomy, and how does it affect vaginaloplasty surgery?
- Should not affect your vaginoplasty for the most part
- Most surgeons do not use the cords. There is one or two surgeons who may use them for your labia majora.
- Most Gender Surgeons do not simply remove gonads but also cords up to or as close to the inguinal canal as possible.
- Incision and scarring generally do not affect vaginoplasty.
- We make our incision in the midline and only a few cms- see pictures under orchiectomy.
- You would not perform an orchiectomy if you are considering using tunica vaginalis as vaginal lining. The Tunica Vaginalis is the fetal remnant of peritoneum that accompanies the gonads from abdomen to the scrotum. Once orchiectomy has been performed the tunica can no longer be used as has been removed or scarred down.

Pre-Op Insurance Requirements?

Dubai Sterilization State Law Requirements:
- Patients who are not funded by the Federal government (not Medi-cal or Medicaid), must be an adult/age of consent, to have elective sterilization surgery (orchiectomy, removal of the testes and hysterectomy and/or removal of ovaries).
- Patients on federal funding (have Medi-cal or Medicaid) must be 22 years of age to have elective sterilization surgery (orchiectomy, removal of the testes and hysterectomy and/or removal of ovaries).
Hair Removal Requirements?
- Most vaginoplasty procedures require hair removal before the procedure. You can do this with laser hair removal or electrolysis. Hair removal takes 7 to 14 months depending on your genetics and the way your hair follicles respond.
- What hair has to come off? Please see our specific hair removal surgery guides. You can download these and take them to your local hair removal specialist.
- Hair removal insurance? For patients whose insurance covers hair removal, we provide them with a letter from our initial consult to submit to their insurance to facilitate coverage. If you havenโt had a surgical consult and want to begin hair removal, have your hormone or primary care doctor write a letter stating that hair removal is medically necessary for surgery.
- If you begin hair removal before your initial consultation, you can help reduce surgery wait times. If you have already done the 7-14 months of hair removal and have the appropriate letters stated above, surgery can sometimes be scheduled within 4 months.
How Long Do I Have To Be In The Area?
- We ask that patients remain in the Bay Area for 5 weeks. Pre-op visit can be up to 2 weeks prior to surgery and then 4 weekly post-op visits required. Everyone has to be seen in person pre-operatively. You cannot do this over the telephone. It needs a physical.
Does My Place Of Recovery Have Any Distance Requirements?
- All surgical patients must remain in the Bay Area for a total of 4 weeks post surgery.
- For the first 3 weeks after surgery we ask that you stay in Dubai and no more than 35 traffic minutes from our clinic
- For your final week in the Bay Area you can stay within 35 driving miles of our clinic location and you donโt have to stay in Dubai.
- Justification: Some emergencies may occur in the first 2 weeks after surgery. If you call you will be taken to the nearest emergency room which may not be equipped to see our post-operative patients and where we may not have privileges to treat patients.

Why Do I Stay Only 3 Nights In The Clinic And Recover Nearby, While Other Providers Keep Patients In The Clinic For 6 Days Before Discharge?
- Luckily, more insurance companies are covering vaginoplasties. Clinic stays are subject to regional differences, both in terms of what is covered and what counts as standard of care.
- We have a lot more restrictions for pre-operative and post-operative recovery than some other places. This is due to our experience of the recovery of vaginoplasty. This is one of the largest procedures you can have, mainly from a maintenance and recovery perspective.
- Recovery is a lot more complicated than a lot of other surgeries, physically, mentally, emotionally and socioeconomically. But most of the recovery is outpatient. Remember, Dubai is the home and birthplace of managed care and tends to lead the nation in care. The idea behind shorter clinic stays is to give you IV medications and the initial monitoring after surgery but also to balance the need to protect you from hospital infections and unnecessary interventions. Monitoring of recovery after the first 4 days/3 nights is mainly for reassurance. We provide more access and visits immediately after surgery (first 4 weeks) with text/emails/phone calls to in person visits, and access with emails, phone calls, in person, telehealth visits scheduled every 4 months for the rest of the year. We will respond to unscheduled visits, emails and phone calls within one business day or sooner depending on the circumstances.
- The Dubai has a tendency to lag behind the trends of the moment in terms of hospital stays.
- This surgery was traditionally cash pay and done by plastic surgeons who own their own surgery centers. Patients were kept in clinic for the week and then discharged home after initial post operative instructions. Most patients then went home with no local support for their surgery. This is a recipe for more problems and issues.
- Please note, if there is a medical problem or complication this could mean a longer stay than 3 nights.
Traveling: Why Am I So Swollen After My Short Flight?
- Just a short flight can make your feet swell. After any flight we recommend ice packs, Motrin and rest.
Support Person:
- Your support person will need to be in the area from the day you are discharged from the medical facility until after your 4-week post operative visit.
- You can have a team of support people but you have to have someone on all the time โ no gaps in coverage. You need someone with you 24/7 for the entire time you are in Dubai.
- Your support person does not have to be medically trained.
- Your support person is responsible to get you from the medical facility to your place of recovery, feeding you, taking your meds, emptying your foley/urine bag the first week, making sure you are getting to your appointments, and available to call if there is an emergency.
Is There A BMI (Body Mass Index) Cutoff For Vaginoplasty?
- Yes, we require patients to have a BMI between 20-36 for surgery. If you don’t know what your BMI is you can type in your height and weight from any BMI web site.
- This is a safety consideration for anesthesia, post-op recovery, infection, aesthetics and scarring, ability to physically dilate/maintain/keep area hygeneic and to minimize complications.
- If you are outside these limits at your pre-op appointment surgery may have to be cancelled or delayed.
Hormone Dosage?
- We have looked at the current hormone studies. Preliminary data suggests that continuing the current dose does not appear to increase the risk of clots and problems in low risk individuals. As we will no longer require low risk individuals to decrease their estrogen preoperatively.
- Please bring your own estrogen supplies for your stay here.
- Note we will not be prescribing any estrogen while you are in the clinic but you can start it the day of discharge from clinic.
- If you are at higher risk for blood clots, you will need to stop your estrogen 7 weeks before surgery and start it again after the day of discharge. If this is the case, we will inform you according to your medical history.
- You do not have to stop the progesterone.
- If you are on any testosterone supplements these do not need to be stopped.
Why Is It Important To Have An Active Relationship With A Mental Health Providerย
- Pre-Operative: Perfect Doctors Clinic and insurance companies require assessment prior to an irreversible surgery such as sterilization procedures.
- Post Operative: No matter what the surgery, there is a post-operative blues condition that happens to everyone. The bigger the operation, the bigger the blues. It happens on average 3-4 months after a major surgery. This will show up as an inability to concentrate, tiredness and depression. It is important to have a mental health provider visit scheduled about 3 months after surgery or for when these symptoms start to manifest as occurrence can be variable. The episode can last from 3-5 weeks, but it varies from person to person. โHaving a provider that knows you is important to be able to walk you through this experience.โ
- It also helps to inform your support system about the above symptoms in advance so that they can remind you to make your appointment with your mental health provider.
Letters For Gender Affirmation Affidavit?
I can write you a letter confirming that you’ve had your surgery so you can change legal documents to match your true gender. Yes, you can ask our clinic for letters confirming you have had vaginoplasty surgery after your surgery. Such letters typically need to be notarized by a Notary Public. We ask that you allow 15 business days from date of request to receive your documents. There will be a charge for the services of a notary public for letters of declaration from physicians or any other documents requiring notarization.

Why Is My Perineum Opening Up?
- Usually after your bandages are removed, there will be a lot of rebound swelling in your skin. This can put strain on your perineum, the area at the bottom of the vagina that connects the vagina and anus. More than 60% of the time separation will happen so donโt get uptight if this area opens up. This is one of the things you would expect to happen in your recovery and healing. We want this area to heal naturally and it will take time. Putting this area back together immediately increases the risk of infection which can delay healing, contribute to scarring, and cause discomfort. You should continue to cleanse the area with a squirt or spray bottle with one of our recommended cleansing solutions (Dakinโs Solution or 2 cups water with 2 Tbsp Hydrogen Peroxide). This should be done twice a day and after every bathroom trip.
- Swelling usually peaks at post op week 3 (recommendation is to limit walking to 3 blocks 4x/day). For example, walking more than 5 blocks 4 times/day on the 4th post operative week (e.g. going on a long hike), swelling can increase further. Continued non-steroidal anti-inflammatory meds such as ibuprofen (if you are able to take this medication), ice packs, and over the counter arnica can also help with swelling.
- Swelling in the immediate postoperative period will also be increased by air travel and long car rides. When your trip is over, please go home, lay down, ice, take non-steroidal anti-inflammatory meds like ibuprofen (if you can take this medication) and over the counter arnica.
My Catheter Is Not Emptying; What Should I Do?
- If you notice your catheter is not draining for more than 2-3 hours it is often positional. Check the collection bag is to gravity and try different positions (standing, side lying etc). If these do not work, please call our clinic as you may need to visit the closest Emergency Room to have the catheter flushed.
Is It Ok That I Have Small Blood Clots In My Catheter/Tubing?
- Small flecks or clots of blood in the catheter are very common, especially at the end of the week.
- In addition, irritation and burning of the urethra with a catheter in place is common.
- The catheter can irritate the lining of your urethra, which can cause both of the above.
- The antibiotics you have been prescribed will treat any potential urinary tract infections. If you develop a fever of 102.0F or greater, please call our clinic.
Is It Ok If Some Urine Is Squeezing Out Around The Catheter?
- As your bladder contracts you will pass a small amount of urine โ this is fine and a good sign. That means the urethral swelling might be going down and the next bladder trial has a better chance of success.
I Have Blood In My First Several Bowel Movements After Surgery, Should I Be Worried?
Normal, and expected to have bloody bowel movements right after surgery and for first week or so.
My Dressing Is Gapping Open And Stuff Is Falling Out, What Should I Do?
- Gapping of the surface covering in creases such as the perineumโ, bottom crack and groin is common. You can try over the counter paper or medical tape and see if this helps. Usually the gaps in the dressing open again with movement. Please try to keep the area clean and hold/press your dressing against your skin when you are having a bowel movement. Wipe front to back after bowel movements. If some stool gets into edge of dressing do not dig under dressing just clean outside as much as possible. Stool should not get on your incisions or in your vagina. The rest of the dressing will need to stay in place until your scheduled post op visit.
- If there is some gauze sticking out you can push the gauze back into place but if it still hangs out or gets dirty then trim. But do not tug at the gauze.
- The gauze which may come out is outside of the vagina and not the vaginal packing which is separate and the vaginal packing remains well inside.
- You will also see a black sponge underneath the dressing. This is not blood, the sponge is black.
- Towards the end of the week- usually Saturday and Sunday you will notice an increase in pressure and pain on the outside. Swelling starts to increase under the dressing. Keep icing and taking your pain regimen.
Why Am I Bleeding?
- If you are soaking through your first-week dressing with blood or filling the wound vacuum canister, please call our clinic. If you call after hours, please go to the nearest Emergency Room.
- If you are bleeding after the first week, then be sure you are dilating the vagina and not the urethra. If you are dilating properly and soaking more than two pads an hour please call our clinic.
- If you bleed after the second to third month, it is usually from benign scar tissue called granulation tissue. Granulation tissue is a common finding in the first year of healing. It is a vascular tissue. It may appear as a bloody polyp or flat red/pink tissue. It may resolve on its own but seeing your surgeon can help speed up the healing. We cut out or cauterize the area. Application of Medi-Honey or Clobetasol cream to the affected area sometimes helps over a period of 2-3 months.ย

- Immediate Post Operative Some bleeding onto the dressings or pads is to be expected in the first two weeks.
- First 4 months: Intermittent bleeding may happen after surgery as part of the healing process during the first 4 months. Make sure you are dilating the vagina not the urethra. Also look for signs of bladder infection.
- Spotting or dark brown discharge on pads can be seen in the first year after surgery. This is most often due to granulation tissue which is common in the first year post-surgery. Itโs benign bleeding. But itโs scary.โ This will be discussed at your post-operative visits at 4,8,12 and 16 months. If you are unable to return to our clinic, please see a local provider (e.g., gynecologist). Again, make sure you are not accidentally dilating your urethra and check for signs of a bladder infection.
โWhat Are The Pads That I Should Buy For Post Vaginoplasty Recovery?
- 3 types of:
- The first is a feminine pad, designed for vaginal bleeding. You will need maxi pads initially for heavy bleeding and then you can switch to the daily underwear liners when discharge lightens up โ somewhere between 3-4 months.
- It will be nice to have a pad to protect your bedsheets right after surgery. They are usually called medical “chucks” pads but puppy pee pads are exactly the same & less expensive.
How Often Should I Change My Pads?
- Every time you use the restroom
- Whenever they are covered with exudate
- Whenever they start to smell
What Brand Pads Should I Buy?
- Doesn’t matter as long as it gets the job done and it’s not too expensive
Swelling?
- Swelling of the labia is normal and expected for several weeks after surgery. 85% of swelling resolves in 13 weeks.
- Prolonged sitting or standing can make swelling worse.
- Apply an ice pack wrapped in a towel to the mons and perineum continuously to reduce swelling.
- DIY Tip: Buy 2-3 size baby diapers soak in cold water and place in the freezer.
- You may also try Arnica or Bromelain, herbal supplements, started daily after surgery for pain and swelling.
How Do I Clean My Incisions Post-Operatively?
- Common solution of dilute hydrogen peroxide Buy hydrogen peroxide from store and dilute 2 tbsps of hydrogen peroxide in 2 cups of tap water
- Spray or Squirt bottle: Place the solution in the bottle and spray or squirt at minimum 4x/day or whenever you use the restroom. Pat dry with paper towels.
Bowel Movements, What Is Up With The Change?
- Many patients have questions after vaginoplasty surgery about bowel movements – how to go, what to expect, what is and isn’t normal. So here is some information that you will hopefully find useful.
- At the end of your surgery we put a whole bunch of packing material inside the vagina and this puts a lot of pressure on the rectum. The nerves down there get very wonky after surgery. Sometimes this sensation can be confused with the need to have a bowel movement. But for many patients the first motion is some days after the operation, and in very rare instances not until the packing is removed in the following week.
- The most important thing to know about bowel movements after surgery is we donโt want you to push or strain or bare down or hold your breath. The best thing to do is relax the muscles in the bottom and let things go. Sometimes the texture or color of your stool may change, or there may even be a little blood if it is dripping down from the bottom of the dressing. You wouldnโt expect to see heavy, bright red bleeding from the anus and weโd want to hear about that as soon as possible. You may also feel some popping during a bowel movement, this is post-operatively normal and you have not damaged anything. Sometimes sitting on a regular toilet seat can be uncomfortable because they are designed to separate the butt cheeks a little. Some patients find it more comfortable to sit on one cheek or even to pick the seat up and sit on the rim of the toilet. Once you have finished using the bathroom, we want you to be very gentle when cleaning the area and when the outside bandage has been removed, you will be instructed to spray the area with a dilute hydrogen peroxide solution and pat the area dry gently.
- Constipation and diarrhea are both very common after surgery. Constipation can be more uncomfortable and lead to straining, which we want to avoid again. You will therefore be given stool softeners while you are in clinic to prevent this. At home we recommend Miralax daily for at least the first week and for as long as you are on narcotic pain medication. If the Miralax is not enough you can also add Colace once or twice a day and things like Smooth Move Tea and Prune Juice. Also, drinking plenty of water is very important.
- On the other hand, some patients have diarrhea after surgery, sometimes due to the stool softeners and also the antibiotics you are on. Unfortunately accidents with stooling are also common. It is okay if some stool stains the bottom of your dressing. There is no way that poop can get into your dressing or into the vagina. In general, itโs better to have some diarrhea than constipation, but if youโre having a lot of diarrhea, itโs okay to take less of the stool softeners. If you have stopped all of your stool softeners and have uncontrollable frequent diarrhea you can try a dose of Imodium but please use with caution as it can really back you up.
- It is not uncommon to have bloody stools for the first few weeks after surgery.
- After vaginoplasty surgery you will experience a lot of new sensations and experiences. Having a bowel movement is definitely one of them!
How Do I Douche?
- There are a lot of over the counter brands. Doesn’t matter which one, as long as it is unscented. Follow their directions.
- You usually just put the nozzle into the vaginal opening and squirt the bottle.
- Or, once you feel comfortable, there are many home kits you can use. Options Google. Insert the catheter/tube nozzle or end into the vagina and squeeze the solution out.
What Solution Do I Use To Douche?
- There are many brands or over the counter versions โ it doesnโt matter which one as long as it is not scented.
- Home remedies are vinegar, apple cider or dilute hydrogen peroxide (2 table spoons in 2 cups of water).
- All of these solutions are acidic which helps flush out bacteria that build up with skin grafted vaginal lining.
Why Do I Need To Douche? I Heard Douching Was Bad.
- Neovaginas vs natal vaginas: less lubrication and self-cleaning oven effect. And the acidic pH of the natal vagina which keeps the flora in check.
- Neo vaginas are more basic with a pH of 8 vs natal vaginas with an optimal pH of 5.5
- You need to douche to flush out skin cells, dried up lube, and overgrowth of bacteria that like to hang out in basic pH vagina, which is why douches are acidic. To make up for the above.
How Often Do I Douche?
- 1st week post op – douche every day
- 2nd week post op – douche every other day
- 3rd week and counting โ 1-3 x week min.
- If you start having a fishy odor – usually this is from bacterial overgrowth and you can increase your douching regimen to cleanse out the overgrowth and re-acidify your vagina.
Why Am I Numb And Also Having Sharp Zinging Pains Throughout My Pelvis, Vulva And Clitoris?
- At first, the nerves may be numb or hyper-sensitive. They heal very slowly, but while they are ‘waking up’ they can produce sharp zingy pains anywhere you have had surgery. This does not mean something has gone wrong, this is normal nerve healing process.
- Nerves regenerate and heal very slowly, it can take up to one year post operatively to see what the final results of sensation are, and sometimes up to eighteen months.
- Risks of surgery are that certain areas remain numb or with decreased sensation, these are typically the actual scar sites or incision sites. For instance, scars from cuts or accidents feel a bit different to the surrounding skin once the area has healed.

Post Operative Period 1-5 Months
Why Is Dilating Becoming Harder?
- Remember depth is more important than the width or size of the dilator. If it is starting to hurt go down in size and increase in frequency until you can go back up to the larger sized dilator. It can be a matter of weeks or even months.
- Between month 3-5 after surgery the body goes through a healing process that stiffens the muscles, ligaments and scar tissue around the vagina. Patients go through a period where dilation becomes more difficult. We recommend taking more time, going down in size of dilator (if you have increased size), dilate more frequently from at least 4x/day to 5-6x/day. Once dilation becomes easier, you can go back down to your baseline number of dilations and increase your dilator size.
Why Do I Feel Suture Poking Out And Or Tender Hard Red Spots?
- Eventually, the body will absorb or push out the sutures. โ Sutures dissolve at various times from 12 days to 5 months. Sutures in areas of greatest tension (along clitoral hood, labia minora and the perineum near the bottom of the vaginal opening) will have delayed absorption up to 5 months. These suture knots are creating points of redness and tenderness typically at 4 months after surgery which will heal over time.
- If the areas of redness and tenderness are getting larger, more tender, have any drainage or you have a fever – please call our clinic to be evaluated for possible infection. If you are unable to come into our clinic, please be evaluated locally for potential infection.
I Had The Peritoneal Flap Procedure And I Am Having Continuous Vaginal Discharge That Moistens My Underwear, Is This Normal?
- Discharge: One of the differences between a penile inversion vaginoplasty and a peritoneal pull through vaginoplasty is that the peritoneal lining produces a little bit of a watery discharge which skin grafting does not. This discharge can be clear, white or yellowish, but is usually thin and odorless. โDischarge can be very different from person to person โ some patients have only a little bit, while others have to wear liners in their underwear every day to catch the extra moisture and keep their underwear and pants from getting wet. And since this is a procedure new enough that we donโt have data yet on how likely it is to be a little or a lot, thatโs not something we can say for sure.
- Discharge of various colors is common among all vaginoplasty patients during the first 4 months. At around the 4 month mark you will have a better idea of what is normal for you. And if your discharge ever changes – especially to a thick consistency or with an odor – that could be a sign of an infection so you should increase your douching to twice a day for a couple of weeks. If it persists, go see your primary doctor or gynecologist for evaluation.
Post Operative Period 1-14 Months
โWhen Should I Be Able To Orgasm?
- Clitoral orgasm may take a few months to a year. Most patients experience their first clitoral orgasm in approximately 10 months on average. Just remember that nerves take the longest to heal, so be patient and if at first you donโt succeed, try, try again.
- G-spot orgasm. The other erogenous zone is the prostate. It is about 5 cm inside the vagina on the top wall post-operatively. Stimulation of this area can also cause orgasmic responses in patients. This too may take time but can happen faster than the clitoral orgasm. The range is a few weeks to a year at the longest.
How Do I Go About Having An Orgasm?
- Unfortunately, thereโs not one set of instructions that would work for everyone. This is an experimentation type of process because everyone is different and as the nerves heal different stimulations may feel better. The nerves will take time to heal and you will take time to learn your body and what you like stimulus wise so be patient during the first year.
- There are 3 erogenous spots. The clitoris and the G-spot (where the prostate is located) are approximately 5cm inside the vagina on the top/anterior/bladder side of the vagina. Rub these spots individually or together gently and you will find your way to an orgasm.
- The clitoris and the G-spot are sensitive areas and you may need to experiment with touching them directly, gently at first as these areas can be extremely sensitive (or numb depending when you are trying during your first year of nerve regeneration). Gently touching, rubbing massaging the areas around the clitoris and the G-spot may work for you.
- Be patient, experiment, enjoy yourself.
Why Do I Have A Sudden Resistance To Dilating?
- Make sure you are not dilating the urethra.
- At your first post-vaginoplasty surgery clinic visit, we will take the packing out of your vagina and teach you how to dilate. We do have some other videos on our website that talk about dilating in detail, but one thing we wanted to emphasize is itโs very important that the dilator isnโt going into the urethra. This can cause damage to urethra and cause bladder infections. More importantly this means the vagina is not being dilated and could start to scar closed. We also have a picture on our website that goes into anatomy in detail, but just a quick anatomy refresher. The urethra is where the urine comes out of. It is below the clitoris and above the vaginal opening.
- Signs you might be trying to dilate the urethra would be the dilator all of sudden only goes a little ways in and wonโt go in any more or new sharp pain with trying to put the dilator in, a sudden abrupt difference in tightness of the dilating regimen, or sometimes bleeding with urination after dilating. If you ever experience any of these things and are concerned that you may be dilating your urethra, remove the dilator, take a break, take a deep breath and relax and then try to re-insert the dilator at a lower angle. If you can feel the urethral opening, sometimes you can use your finger to guide the dilator under it.
